All students trying out for a school sport are required to physically turn in a physical/medical history form and also complete the online athletic participation packet (APP) prior to the start of each school year. Student-athletes will not be allowed to participate in any athletic classes, practices or events until all necessary forms are completed and processed with the MISD Athletic Department. Physical exams must be signed by a licensed physician, physician’s assistant or advanced nurse practitioner.
